Contract for Medical PPO Health Care Benefits

Re: Contract for Medical PPO Health Care Benefits
requesting authorization for the Purchasing Agent to enter into a contract with BlueCross BlueShield of Illinois, Chicago, Illinois, for medical PPO health care benefits for all eligible Cook County employees, Elected Officials and eligible dependents. A Request for Proposal (RFP) was prepared and issued for medical PPO health care benefits as negotiated per collectively bargaining agreements. Proposals were submitted from six (6) companies. After reviewing all responses, the following companies were selected as finalists:
BlueCross BlueShield of Illinois
Cigna HealthCare of Illinois
Unicare Health Plans
The contract will be effective December 1, 2004 to November 30, 2005. In addition, contract provisions will include two subsequent twelve-month renewal periods to coincide with the upcoming labor negotiations, allowing for plan design changes as needed.
Estimated Fiscal Impact: $28,240,000.00 to be pro-rated among the 490-176, 499-176 and 899-176 Accounts based on actual enrollment. Contract period: December 1, 2004 through November 30, 2005.
Sufficient funds have been appropriated to cover this request.

requesting authorization for the Purchasing Agent to enter into a contract with First Commonwealth, Inc., Chicago, Illinois, for managed care dental benefits for all eligible Cook County employees, Elected Officials and eligible dependents. A Request for Proposal (RFP) was prepared and issued for managed care dental benefits, for both HMO and PPO services as negotiated per collectively bargaining agreements. Proposals were submitted from six (6) companies. After reviewing all responses, the following companies were selected as finalists:
BlueCross BlueShield of Illinois
Delta Dental of Illinois
First Commonwealth, Inc.
