requesting authorization to accept a grant award in the amount of $205,203.00 from the Illinois Criminal Justice Information Authority (ICJIA). This grant was made available through the Federal Anti-Drug Abuse Act, and will fund gender-responsive treatment and aftercare services for adult female offenders sentenced to probation for substance abuse-related offenses.
The court’s Adult Probation Department administers the community-based transitional services for Female Offenders program funded by the grant, which helps female offenders to make the transition from custody under the Sheriff’s Department of Women’s Justice Services to probation supervision. Services provided include drug assessment, detoxification services, counseling, and life skills development. The program’s primary focus is to break the cycle of addiction and crime, and facilitate probationer’s successful reintegration into the community. Program outcomes will be carefully measured in compliance with the department’s evaluation protocols.

COOPERATIVE EDUCATIONAL MASTER AGREEMENT PROGRAM ADDENDUM

requesting authorization from the Cook County Board of Commissioners to increase and extend the existing Program Addenda (Subagreements) to the Cooperative Educational Master Agreement (CEMA) (current CEMA expiring 6/30/05) Contract No. 01-43-450 as authorized by the County Board on December 5, 2000 with the University of Illinois at Chicago, Chicago, Illinois, for Stroger Hospital of Cook County, Department of Surgery, Division of Orthopaedic Surgery, Division of Plastic Surgery, and Division of Otolaryngology. The extension of the existing subagreements is requested for seven (7) months through June 30, 2005, concurrent with the expiration of the existing Master Agreement.
The Program Addendum will combine the academic and clinical resources of the University of Illinois Hospital and Medical Center to the educational and training experience of Stroger Hospital of Cook County. Pursuant to the subagreement, the University of Illinois Hospital and Medical Center will provide rotating residents to include:
- A maximum of 7 residents - PGY 2 to PGY V - Assigned concurrently to Orthopaedic Surgery three (3) month rotations at all times during the academic year.
- Three (3 FTE) residents, PGY VI-VIII, assigned concurrently for coverage in three (3) month rotation block in Plastic Surgery with assignments in Burn and Hand Services.
- Three (3 FTE) residents, PGY III-PGY V, shall be assigned concurrently throughout the academic year in the Division of Otolaryngology in three (3) month rotations.

requesting authorization for the Purchasing Agent to enter into a contract with Cole Managed Vision, Twinsburg, Ohio, to provide Cook County employees and their dependents with a Vision Care program from December 1, 2004 through November 30, 2007.
Based on an extensive financial analysis and overall evaluation of benefits from the respondents, I recommend the selection of Cole Managed Vision. Cole Managed Vision offers the following advantages: lowest costs for examinations, spectacle lenses, contacts and frames; largest provider network in Cook County with 140 locations; high level of satisfaction for Cook County employees since 1995; responsive service and ease of administration to covered members; very competitive administrative fee; and a three-year rate guarantees for materials and services.
All fees and expenses will be paid from the following accounts:

The actual charges for each fund will vary based on the actual monthly utilization. Sufficient funds have been appropriated for these expenditures.

WHEREAS, the salary of the State’s Attorney, an Officer of the County of Cook, has been increased to $163,915.00 annually, effective July 1, 2004, pursuant to the Compensation Review Board of the State of Illinois governing annual cost-of-living adjustments.
NOW, THEREFORE, BE IT RESOLVED, that the new salary of the State’s Attorney of Cook County, as amended in accordance with aforesaid Resolution, be approved by the Cook County Board of Commissioners.

WHEREAS, an Interest Arbitration Award has been issued concerning unresolved salary issues covering the period December 1, 2003 through November 30, 2006.

Resolution Approving Class 8 Special Exception to Abandonment
respectfully submitting this Resolution regarding the company’s application for Class 8 property tax incentive for your consideration.
Jan Szwanjos requests approval of the special exception to the 24 month abandonment rule under the Class 8 Ordinance. The Resolution is required so that the company can complete its application to the Assessor of Cook County.
Submitting the Jan Szwanjos application for Class 8, the Resolution of the City of Harvey, and a Department of Planning and Development staff report for your information.
PROPOSED RESOLUTION
WHEREAS, the Cook County Board of Commissioners has adopted a Real Property Assessment Classification 8 which provides an applicant a reduction in the assessment level for an industrial facility; and
WHEREAS, the County Board of Commissioners has received and reviewed an application from Jan Szwanjos, and the Resolution from City of Harvey for an abandoned commercial facility located at 501 East 163rd Street, Harvey, Cook County, Illinois, Property Index Number 29-21-116-006-0000; and

WHEREAS, Class 8 requires the validation by the County Board of the shortened period of qualifying abandonment in cases where the commercial facility has been abandoned for less than 24 consecutive months upon purchase; and
WHEREAS, the Cook County Board of Commissioners has determined that the building was abandoned for ten (10) months at the time of purchase, and that special circumstances are present.
NOW, THEREFORE, BE IT RESOLVED, by the President and Board of Commissioners of the County of Cook, State of Illinois, that the President and Board of Commissioners indicate their approval for the filing of an application for a Cook County Real Estate Classification 8 abatement incentive to Jan Szwanjos, for an abandoned commercial facility located at 501 East 163rd Street, Harvey, Cook County, Illinois; and
BE IT FURTHER RESOLVED, that the County Clerk is hereby authorized and directed to forward a certified copy of this Resolution to the Office of the Cook County Assessor.
