DENNIS MANZKE, Chief of the Administrative Services Bureau, State’s Attorney’s Office

 

requesting authorization to accept a grant award in the amount of $302,711.00 from the Department of Justice, Office on Violence Against Women.  This grant will enable the State’s Attorney’s Office to continue to contract with Life Span Center for Legal Services and Advocacy and Hull House Association to provide services to high-risk victims of domestic violence under the State’s Attorney’s Target Abuser Call (TAC) Program.  Specifically, grant funds support the salaries and fringe benefits of two (2) domestic violence court advocates from Hull House and one (1) civil staff attorney from Life Span. 

 

There is no match contribution required for this grant.

 

The authorization to apply for this grant was given on January 23, 2003 by the Cook County Board of Commissioners.

 

Subcontract with Hull House Association:  Estimated Fiscal Impact:  None.  Funds provided through the Office on Violence Against Women:  $175,062.00.  Subcontract Period:  September 1, 2003 through August 31, 2005.

 

Subcontract with Life Span Center for Legal Services and Advocacy:  Estimated Fiscal Impact:  None.  Funds provided through the Office on Violence Against Women:  $106,579.00.  Subcontract Period:  September 1, 2003 through August 31, 2005.

GRANT AWARD RENEWAL

 

ITEM #7

 

 

Transmitting a Communication, dated October 27, 2003 from

 

DENNIS MANZKE, Chief of the Administrative Services Bureau, State’s Attorney’s Office

 

requesting authorization to renew a grant in the amount $123,600.00 from the Illinois Criminal Justice Information Authority (ICJIA).  This grant provides a fourth year of continued funding for the Youth Gun Violence Program.  This program serves juveniles who have been charged with first and second-time nonviolent firearm offenses with the goal of preventing future gun violence.  The program is a partnership between the State’s Attorney’s Office, the Cook County Circuit Court’s Juvenile Probation Department and Schwab Rehabilitation Hospital and Care Network.  The program funds a full-time weapons data specialist within the State’s Attorney’s Office who coordinates program components and tracks weapons charges in juvenile court.  Schwab Rehabilitation Hospital and Care Network functions as the service provider and facilitates program elements.

This grant requires that the office match one dollar for each three dollars of federal funding.  The match commitment for this program is a cash match that funds the fringe benefits and a portion of the salary for the state’s attorney’s weapons data specialist.

 

The authorization to apply for this grant was given on July 1, 2003 by the Cook County Board of Commissioners.

 

Estimated Fiscal Impact:  $46,675.00 (Required Match - $41,200.00 and Over Match - $5,475.00).  Grant Award:  $123,600.00.  Funding period:  October 1, 2003 through September 30, 2004.  (250-818 Account).

 

Subcontract with Schwab Rehabilitation Hospital and Care Network.  Estimated Fiscal Impact:  None.  Funds provided through the Illinois Criminal Justice Information Authority (ICJIA):  $103,417.00.  Subcontract Period:  October 1, 2003 through September 30, 2004.

 

COOPERATIVE EDUCATIONAL MASTER AGREEMENT PROGRAM ADDENDUM

requesting authorization for the Purchasing Agent to enter into a Program Addendum to the Cooperative Educational Master Agreement between Loyola University Medical Center and Stroger Hospital of Cook County, Contract No. 94-43-1236, approved by the Cook County Board of Commissioners on August 9, 1994.

 

This Program Addendum allows Loyola University Medical Center radiation oncology residents to rotate through Stroger Hospital of Cook County.  Approximately two (2) residents per year would rotate for two (2) months each.  This rotation would supplement the gynecological oncology experience for radiation oncology residents of Loyola University Medical Center.

 

Estimated Fiscal Impact:  None.  Contract period:  November 1, 2003 through October 31, 2004.  Requisition No. 38900762.

 

AGREEMENT

 

ITEM #9

 

 

Transmitting a Communication, dated October 21, 2003 from

 

LACY L. THOMAS, Chief Operating Officer, Stroger Hospital of Cook County

 

requesting authorization for the Purchasing Agent to enter into an agreement with Eli Lilly and Company, Indianapolis, Indiana, for the purpose of having Stroger Hospital of Cook County participate in the Zyprexa Indigent Patient Program.

 

Under the agreement Eli Lilly and Company will replace on a monthly basis those Zyprexa medications which were dispensed to qualified patients whose income is at or below the Federal Poverty Guidelines, and who otherwise lack public or private insurance coverage.  The replacement medications will be shipped directly to the hospital and placed into the operating inventory.

 

The hospital estimates that participation in the Zyprexa Indigent Patient Program will result in an annual cost savings of $205,000.00 for the hospital.

 

Estimated Fiscal Impact:  None.  Contract period:  December 1, 2003 through November 30, 2004.  Requisition No. 48970761.

Reason:

City Colleges of Chicago has developed excellent staff development courses and offers the courses in several locations and/or in cooperation with other community colleges.  City Colleges will work with Clerk of the Circuit Court staff in tailoring the programs to the specific needs of the Clerk’s employees.  Due to the large number of staff attending these training classes, working with the employees schedule and operational needs, training with City Colleges which began in June, 2003, needs to be extended through August 31, 2004.  The expiration date of this contract is December 31, 2003.

Reason:

These vendors provide court-ordered group intervention counseling services to adult criminal offenders sentenced to probation for acts of domestic violence and other related offenses.  Counseling services are an integral part of a series of program interventions designed to stop offender violence and intimidating behaviors.  Other program interventions include intensive probation supervision, education, job training, and cross referrals to substance abuse treatment, mental health services and other programs.  Depending upon the nature of the conviction, the Social Service Department supervises program participants.

The court sought proposals from providers of these services through a formal Request for Proposal (RFP) process in 2002.  These vendors were recommended after careful consideration of each proposal, based upon service criteria.  Each vendor has been approved by the Illinois Department of Human Services, Domestic Violence Advisory Council, pursuant to the Illinois Protocol for Partner Abuse Intervention Programs.  Additionally, each vendor has prepared a service plan, a realistic and cost effective budget, and has provided sufficient evidence of professional expertise necessary for these services.

 

Estimated Fiscal Impact:  $290,000.00.  Contract period:  December 1, 2003 through November 30, 2004.  (541-260 Account).
