Registering An Assumed Name

Step 2 - Submit Legal Notice

A Copy of a Legal Notice to be Published form must be completed and returned with your Assumed Name Certificate form.

How to submit the Legal Notice to be Published

  1. You should receive the legal notice form with your application in the mail, or you may download and print the Copy of Legal Notice to be Published* online.
  2. List your assumed name.
  3. List your exact business address.
  4. List the names and addresses of all business owners.
  5. Mail or deliver the application, along with the Legal Notice and the fee to the County Clerk's office.

  6. After you file the application and Legal Notice and pay the fee, the Clerk's office will issue you an Assumed Name File Number and return to you the Legal Notice with the file number included.
  7. After the Clerk's office returns the Legal Notice and file number to you, your business must publish the notice in a newspaper.
